By submitting this MRI order form, you agree to the following terms and conditions:

1. Medical Disclaimer

• This MRI service is provided for informational and diagnostic purposes only. It does not constitute a diagnosis, medical treatment, or patient-doctor relationship between you and Dr. Franz Jones or any affiliated provider.

• You acknowledge and agree that it is your sole responsibility to obtain the MRI results and seek appropriate medical consultation for interpretation and follow-up care.

2. Hold Harmless and Liability Waiver

• You agree to hold harmless, release, and forever discharge Dr. Franz Jones, his estate, his heirs, successors, and assigns from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by you or any third party in connection with the use of this MRI order form or any services rendered.

• You also agree to hold harmless and release Get MRI Now, PLLC, including its owners, members, managers, employees, contractors, and agents, from any and all claims, liability, or damages resulting from:

  • Any alleged failure to diagnose or treat a medical condition;

  • Delay in obtaining or acting on results;

  • Misinterpretation or misuse of the results by any third-party healthcare provider or yourself.

3. No Right to Sue

• By using this form, you waive any right to bring a lawsuit or legal claim against Dr. Franz Jones, his estate, heirs, or Get MRI Now, PLLC. This waiver applies to all legal theories, including but not limited to negligence, breach of duty, or any tort or contract theory.

4. Obligation to Follow Up

• You agree and acknowledge that it is your responsibility to:

  • Obtain your MRI results in a timely manner;

  • Follow up with a licensed medical provider regarding the findings;

  • Seek further diagnostic tests or treatment as needed.

5. No Emergency Services

• This form and the services associated with it are not intended for use in emergency situations. If you are experiencing a medical emergency, you must call 911 or seek immediate medical attention.

6. Consent

• By checking the acknowledgment box or submitting the form, you confirm that:

  • You have read and understand the above terms;

  • You are voluntarily agreeing to all of the above without coercion;

  • You are legally competent and over the age of 18.
